Job Title: Social Work Case Manager Spring/Summer Intern

Job Description: Lakeland Care Services is seeking a Social Work Case Manager Spring/Summer Intern to join our team. The Social Work Case Manager Spring/Summer Intern will work closely with our team to provide support to eligible adult clients (age 18+) in their transition into housing, increase long-term stability in the community, and avoid future periods of homelessness or institutionalization.

Responsibilities:

• Provide community-based Housing Stabilization Services to eligible adult clients (age 18+)
• Assist in developing person-centered housing plans with each client
• Identify and assist with resolving barriers to accessing housing
• Support the person in applying for all benefits that will help them afford their housing
• Research and contact prospective housing options for availability and information, assist the person with applying for housing
• Support the person to understand and maintain income and benefits necessary to retain housing
• Maintain a caseload of at least five clients as assigned, meeting productivity guidelines, and proactively seeking referrals as needed to maintain a full case load
• Maintain billing and documentation in accordance with Department of Human Services guidelines
• Abide by and comply with the rules of confidentiality, Data Privacy Act, mandatory reporting laws, HIPPA requirements, and organizational policies and procedures
• Participate in supervision, under the direction of designated supervisor
• Perform other duties as assigned

Requirements:

• Currently enrolled in a healthcare, social work, or human resources program at an accredited college or university
• Strong communication skills (written and spoken) with co-workers, clients, and outside parties
• Ability to work independently with minimal direction and within part of a team
• Experience with computerized management information systems and capable of entering client sensitive data into electronic data information systems
• Employment is subject to background check/fingerprinting

Schedule:

• Part-Time (20 hours per week)
• Flexible schedule with availability on weekends
• Must be available to start immediately
• In-person first 90 days

Pay:

• $20-22.15 per hour
